Use the ArtQuest Application Process Checklist to make sure that you have completed all parts of the application process. Applications which are not complete will not be processed.

There are five separate pages of forms, in a printable PDF format, to complete in addition to the Other Requirements listed under the Checklist.

To complete the PDF forms download the packet of forms and print them out in Adobe Reader. Then fill out each form and sign and date them where required. Don't forget to submit a copy of all forms.
